Swimming Ability & Water Readiness

I confirm that the participant:

* Can swim independently in deep water

* Can float/tread water

* Is comfortable being in open water and waves

I understand that surfing takes place in a natural ocean environment and that conditions may change at any time.

Safety Risk Acknowledgment

I understand and acknowledge that surf camp activities involve inherent risks, including but not limited to:

• Strong currents and rip tides
• Changing wave and weather conditions
• Uneven ocean floor, rocks, reefs, and sandbars
• Collisions with surfboards or other participants
• Marine life
• Physical fatigue and overexertion

I understand that these risks cannot be eliminated even with proper instruction and supervision.

Participant Safety Responsibilities

I agree that the participant will:

• Follow all instructions given by surf instructors and camp staff
• Remain within designated surfing and swimming areas
• Use required safety equipment (including leash and rash guard, if provided)
• Notify instructors immediately of discomfort, injury, fatigue, or distress
• Refrain from unsafe, reckless, or disruptive behavior in the water or on land

I understand that failure to follow safety instructions may result in removal from activities for the safety of the participant and others.

Instructor Authority for Safety

I acknowledge and agree that surf instructors and staff have full authority to:

• Limit participation based on ocean conditions
• Modify activities due to weather or safety concerns
• Remove a participant from the water at any time if safety is at risk

No refunds or credits are guaranteed if activities are reduced or canceled due to safety concerns.

Medical & Health Disclosure

I certify that the participant is physically able to participate in surf camp activities.

Any medical conditions, medications, allergies, or physical limitations that could affect safety must be listed below:

I understand that failure to disclose relevant medical information may increase risk.

Emergency Care Authorization

In the event of injury or illness, I authorize surf camp staff to obtain emergency medical treatment for the participant if necessary.

I understand that I am financially responsible for any medical services provided.

Safety-Based Release of Liability

In consideration of participation in surf camp activities, I hereby release and hold harmless: EAST COAST HUI SURF SCHOOL AND STAND UP PADDLE LESSONS

its owners, instructors, employees, independent contractors, volunteers, and representatives

from any and all claims arising from participation in surf camp activities, including injuries related to ocean conditions, instruction, supervision, or equipment use, to the fullest extent permitted by law.

This release applies even if such injury results from the ordinary negligence of the released parties.

Acknowledgment of Understanding

I confirm that I have read and understand this Safety Acknowledgment and Liability Waiver.

I understand the risks involved in surfing and ocean activities and accept full responsibility for the participant’s safety-related conduct
